The Most Intimate Nip-Tuck

A 42-year-old Chicago woman with three beautiful children found herself divorced and back in the dating scene again.

"But I was so stretched from childbirth, I felt very little sensation during love making," said the woman who asked not to be identified.

"And to make life worse, I suffered from some urinary incontinence whenever I ran, lifted something heavy, coughed, sneezed or laughed hard."

Vaginoplasty

The woman first visited her doctor because of the incontinence which was caused by the internal drooping down of her pelvic organs. Later, an urogynecologist performed an operation known as a sling procedure and pelvic reconstruction with vaginoplasty to stop the leaking and reduce her bulging bladder. The operation was successful and the patient enjoyed an added bonus: her enjoyment of sex greatly improved because the stretched tissues had been tightened and she felt more friction from the vaginoplasty. Friction leads to higher sexual sensations for couples.

A vaginoplasty is surgery inside the vagina for tightening while the labiaplasty procedure is surgery done outside the vagina, specifically, on the labia minora and majora.

Unfortunately, experts say endless Kegel exercises can not do anything to repair a fallen pelvic floor.

Another fortunate moment often happens when women undergo a standard surgical bladder repair and then return to their gynecologist for follow-up visits, reporting that their sex lives have also improved. That happens because pelvic surgery often can result in bringing together loosened vaginal tissues while removing excess vaginal skin, all of which can result in a narrower vaginal canal and more friction.

Labiaplasty

"A vaginoplasty repairs loose muscles in and around the vagina," says Sam Gershenbaum, D.O., a Florida plastic surgeon who performs labiaplasty, a surgical procedure to trim the labia minora, the inner vaginal lips.

"Many women interested in labiaplasty complain of embarrassment while wearing beachwear or discomfort when wearing revealing clothing like workout togs or tight jeans," says Dr. Gershenbaum.

The cause of enlarged lips can be genetic, hormonal, due to a large weight gain or childbirth. Also, estrogen levels in puberty and in pregnancy often spike and act as triggers for dramatic growth of the labia minora and the start of vulvar irritation.

Some professional women, aerobic instructors, and at least one Sports Illustrated bathing suit model have had the procedure because:

  • Their labia were revealed due to thongs and other skimpy swim wear
  • The tissues became irritated by vigorous activity
  • Speedos, leotards or swimsuits were causing additional irritation

Labia Surgery

Says Edward Jacobson, M.D., a board certified gynecologist and surgeon at the Laser Vaginal Rejuvenation Institute in Greenwich, Connecticut: "My labiaplasty patients have included an equestrian instructor who could no longer ride her horse without great discomfort, many professional dancers, several aerobics instructors and a marathon runner."

Others who seek labia reduction complain that intercourse can cause overly long lips to be painfully drawn inside them.

Some surgeons repeat the vaginoplasty.

"Some patients return, saying the procedure has not made them tight enough," says Dolores Kent, M.D., a Los Angeles cosmetic and gynecological surgeon who says many patients also ask for G-spot augmentation, a procedure sometimes known as a "G-shot."

"I inject collagen into the G-spot to build up the area, making it more sensitive," says Dr. Kent. Some researchers maintain the G-spot is an area within the vagina which is instrumental in causing more pleasurable intimate relationships.

Other experts nonetheless report these claims are anecdotal and that science has not been able to locate an anatomic structure specifically defined as a G-spot. Nonetheless, the demand for the G-shots is increasing.

Vaginal Rejuvenation

"The reality of such personal surgery, "down there" is that the person who sees your genitals is the person who matters the most," says Gary J. Alter, M.D., a Beverly Hills plastic surgeon who is board certified in both urology and plastic surgery. Dr. Alter is frequently seen on television's E! Entertainment hit, Dr. 90210.

Labiaplasty and vaginoplasty are most frequently done in a surgery center or operating room. Time required is usually one to two hours for either procedure. Performing both at once can require three hours by a meticulous surgeon.

Recovery from vaginoplasty and labiaplasty requires two to four days, with most patients being able to return to work shortly thereafter. Most labiaplasty patients can resume intimate relations with their partners in six to eight weeks. Vaginoplasty patients may require an additional two to four weeks of healing to soften and stretch the scars formed by skin closure.

Other frequently requested personal surgeries for women include reduction of the mons pubis, a procedure that surgically reduces the fatty area in the lower abdomen. Common requests include laser resurfacing or fat augmentations of their labia majora. Operations to reduce the amount of hooding around the clitoris are also very common while laser hair reduction within the bikini line is widespread.

On the East and West Coasts, some ethnic groups place a high value on virginity so gynecologists, urogynecologists and plastic surgeons often perform a procedure known as a hymenoplasty, an operation that reconnects the ends of the hymen so that the appearance and feel of virginity is reestablished.
